To complete installation and profit center setup, you will need to download the Installation Setup Form and the Form Customization Instructions. Using the Form Customization Instructions, customize your Installation Form Template before sending it to your customers to be completed.
Please download the Installation Setup Form that relates to the partner program you have selected, i.e. partner bills customer, Shift4 bills customer etc. Please have your customers, their MSP or your staff fill in the Installation Setup Form and one Profit Center (Merchant) Processor Setup Form for each processor your customer will be using. For example, if your customer is using Vital for bank card processing and your customer wants to process directly to American Express, you will need both the Vital form and the American Express form.
These forms are automated Word Templates which allow you to TAB from field to field. A help prompt is displayed at the bottom of the screen to assist you with filling in the fields. Fields are limited as to length and content. If you wish, you can print the forms and fill them in manually. A complete set of instructions for filling in the forms is available for download.
Forms completed in MS-Word should be saved as ".doc" files, attached to an e-mail and sent to installations@shift4.com. Forms filled in manually should be faxed to Attn: Installation Coordinator at (702) 597-2499.
